Medical Form (Required Before Training)
PADI MEDICAL FORM.pdf
This is the 2023 RSTC Approved Medical form for Scuba Diving Activities. It must be filled out before any dive training is begun.
A Physician’s Approval is required for any diver who has a potential or actual medical condition—specifically if any question on Page One is answered “Yes,” and the Page Two details indicate a physician’s release is required—for any condition deemed adverse to scuba diving.
